Hospital discharge & reablement support

Focused, short-term support to help you get back on your feet after a hospital stay, illness or a fall — rebuilding strength, confidence and independence so you can do more for yourself again.

What reablement involves

Support that helps you do more, not less

Coming home after a hospital stay or an illness can feel daunting, and it is easy to lose confidence. Reablement is care with a goal: helping you get back to doing things for yourself, step by step.

Rather than doing everything for you, we do things with you, encouraging a little more each week and working alongside any physio or occupational therapy. Most people need us less as time goes on — which is exactly the point.

Goal-led from day one

Clear, personal goals — from making a cuppa to managing the stairs again.

With you, not for you

Encouraging independence rather than taking over, at a safe pace.

Alongside your therapists

Carrying on the exercises and advice from physio and OT between visits.

Confidence rebuilt

Is it right for you?

Signs reablement would help

  • You’re heading home from hospital and want support to settle back in.
  • Confidence has taken a knock after a fall or an illness.
  • Everyday tasks feel harder than they did just a few weeks ago.
  • A discharge team has suggested some support to get back on track.
  • The aim is to regain independence, not to hand it over for good.

Six weeks on, Mum’s making her own tea again. We didn’t think we’d see it.

Questions

The things families usually ask

Anything we haven’t covered, just call — we would rather talk it through than have you guess.

Isn’t the goal to need you less?

Exactly — reablement is short-term and success means you doing more for yourself. We step back as you step forward.

Do you work with the hospital or physio?

Yes — we pick up the discharge plan and carry on the exercises and advice between appointments.

How quickly can support start after discharge?

Often the same day or the next — we know coming home can’t wait, and we’ll do our best to be there.

What if longer-term care is needed?

We make that transition simple — no starting again with strangers or switching provider.
